“Autoimmune Brain Diseases Reconsidered by Psychiatrists”

A new category of autoimmune diseases affecting the brain is prompting psychiatrists to reconsider certain diagnoses. Dr. Chris Hahn, a neurologist at the University of Calgary’s Cumming School of Medicine, highlights that autoimmune encephalitis, while uncommon, can manifest with psychiatric symptoms, leading to potential misdiagnosis as a mental disorder.

Nora Scott from High River, Alberta, initially experienced sleep difficulties and racing thoughts before her health journey commenced. Her behavioral changes, including excessive cleaning and impulsive spending, were initially attributed to bipolar disorder, resulting in a hospital admission and diagnosis.

After four years and a subsequent hospitalization, it was revealed that Scott actually had anti-NMDA receptor encephalitis, a rare autoimmune condition affecting the brain. Dr. Hahn explains the complexities of autoimmune diseases and the challenges in diagnosing conditions like autoimmune encephalitis, which can mimic mental disorders and lead to misinterpretation.

Despite initial misdiagnosis, Scott’s case underscores the importance of accurate diagnosis and treatment. Dr. Jadah Johnson played a pivotal role in identifying Scott’s condition during her second hospitalization, emphasizing the significance of considering alternative diagnoses for atypical symptoms.

National guidelines developed by Dr. Hahn aim to enhance the identification of autoimmune encephalitis cases, emphasizing the rarity of misdiagnosing mental illness when autoimmune encephalitis is the underlying cause of psychosis. While there remains stigma around mental health conditions, advancements in understanding autoimmune origins of psychosis are gradually reshaping perceptions and treatment approaches.

Dr. Jai Shah from McGill University underscores the importance of avoiding oversimplified narratives around autoimmune conditions versus mental illness, advocating for holistic care and destigmatization of mental health disorders. Ultimately, the focus should be on providing effective care tailored to individual needs, fostering hope and recovery for individuals facing complex health challenges.
